The HANOVER FAMILY HEALTH TEAM is inviting applications for a FULL-TIME SOCIAL WORKER The successful applicant will work collaboratively with our dynamic team of interdisciplinary health professionals and physicians to provide primary care for the well-being of our patients. You will be a strong patient advocate who is responsible for supporting patients requiring social services, assessing and treating mental health needs and providing counselling for groups, families and individuals of all ages. Position Requirements: •Bachelor of Social Work or Master of Social Work degree •Current registration in good standing with the Ontario College of Social Workers and Social Service Workers and proof of professional liability insurance •Demonstration of excellent clinical judgement, treatment, case management, program planning, teaching, problem solving and decision-making skills •Recent community based experience preferred •Excellent oral and written communication skills •Proficient computer skills •Ability to work both independently and as a collaborative team member We offer competitive compensation, including HOOPP membership, an excellent work environment and a high achieving, team approach to patient-centred care.
