COMMUNITY CONNECTIONS: HOUSING AND SUPPORT Requires a COMMUNITY HOMES FOR OPPORTUNITY WORKERS 3 Permanent Full-Time Positions – Owen Sound (37.5 hours/week) Our program is operated by HopeGreyBruce Mental Health and Addictions Services. We are funded by the South West Local Health Integration Network and Ontario Ministry of Health and Long-Term Care to provide housing and support for adults recovering from serious mental illness. The Community Homes For Opportunity team will provide community based supportive services for adults with severe and enduring mental illness and complex needs, residing in former Homes for Special Care dwellings. These services will be highly coordinated and intensive but flexible for clients with longer term needs living in the community. The team will be based out of Owen Sound and provide outreach to 3 separate homes, located in Hanover, Meaford and Owen Sound. Working with more than forty individuals, experience in providing one-on-one and group-based skills development is essential. As a member of the Community Homes for Opportunity team, you will provide specialized rehabilitation services based on culturally appropriate therapeutic engagement, identifying unmet needs and collaboratively formulating individual wellness plans aimed at increasing independence in care planning, self-management and identification of recovery goals. Working with individuals to support their personal goals, manage symptoms, carry out recovery-oriented support and promote whole-person wellness in the community is the central focus of this team. QUALIFICATIONS: •University degree or equivalent combination of post-secondary education and experience in an applied Human/Social Service field and five years in a mental health and addictions setting including community work experience are required. Registration in a professional college may become a requirement of this position •Knowledge of local community resources •Firm understanding of harm reduction approach and its implementation in a community based setting •Demonstrated ability in crisis de-escalation skills applied in a team setting •Demonstrated ability to work with individuals who are marginalized, under-served and who may be at risk of homelessness •Demonstrated ability working with individuals living with substance misuse issues and/or an untreated mental illness •Understanding of a trauma informed approach •Knowledge of psychosocial rehabilitation practices •Ability to establish and maintain therapeutic rapport •A sound understanding of the issues related to meeting basic needs and the impact of institutionalization •Ability to work effectively, independently and in cooperation with others •Ability to communicate effectively, both verbally and in writing •Ability to observe and recognize changes in clients •Experience with financial literacy and the ability to assist in the development of individualized budgets •Demonstrated ability to facilitate groups •Experience in assessing individual capacity regarding decision-making and ability to follow protocols in place for those individuals who have been deemed to lack capacity •Demonstrated ability to promote and support individual recovery •Employment in this position requires a valid driver’s licence, the use of a personal vehicle and a satisfactory criminal records check, including a vulnerable sector screening HopeGreyBruce is committed to providing a welcoming and inclusive workplace.
