The Brockton and Area Family Health Team (BAFHT) Is inviting applications for a Registered Dietitian Permanent Part-Time Position 18.75 hours/week (with potential to increase to 37.5) The successful candidate will work closely with local family physicians and allied health providers as part of a strong interdisciplinary health care team at our Chesley and Durham sites. Main Responsibilities •Assist with the development and delivery of health programs (ie. Diabetes, Chronic Disease Prevention) •Complete nutrition assessments, develop medical nutrition care plans with patients, and monitor outcomes •Develop educational resources targeting chronic disease management, health promotion and risk reduction •Program evaluation, accountability, and quality reporting •Develop partnerships with community organizations to provide nutrition programs collaboratively Position Qualifications •Baccalaureate of Science in Nutrition or equivalent required •Registered Dietitian with the College of Dietitians of Ontario •Membership with Dietitians of Canada Association •Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board considered an asset •Demonstrated skills and experience in health promotion and chronic disease management •Proven ability to communicate effectively with patients, families and staff •Experience with coordinating patient care with community resources •Proficiency in the use of computers and information technology.
