The Brockton and Area Family Health Team Is inviting applications for a Social Worker Permanent Full-time Position The successful candidate will work closely with local family physicians as part of a strong interdisciplinary health care team at our Chesley and Durham sites. Position Qualifications: •Masters of Social Work or Counselling degree from a recognized program •Current registration with the OCSWSSW or OACCPP •Recent primary care or community based counselling/therapy experience •Experience working with individuals, families and groups •Demonstrated ability to work collaboratively within an interdisciplinary team •Excellent interpersonal, communication and organizational skills •Proficiency in the use of computers and information technology The Brockton and Area Family Health Team offers an interesting and challenging work environment, the ability to participate in shaping new directions in health care, a competitive compensation package, and a supportive environment.
